Responsibilities
The Registered Nurse collects and analyses patient data, formulates an individualized plan of care, determines progress toward the attainment of outcomes, coordinates care delivery, promotes a safe practice environment, and advocates mutual respect with patients, families, and other healthcare providers
Conducts in-home assessments, and develops, and evaluates the plan of care in partnership with the patient, representative, and caregiver(s)
Communicates significant clinical changes or findings to the physicians involved in the plan of care and other health care practitioners
Educates patients and families on disease management, medication administration, and preventative care
Monitors vital signs and observes patients for any changes in health condition and provides wound care
Collaborates with other healthcare professionals to ensure holistic care for patients
Maintains accurate and up-to-date medical records and documentation timely
Supervises and instructs home health aides
Accepts student nurses as directed
Takes on-call during evening hours, nights, holidays, and weekends on a rotating basis

Duties/Responsibilities

Education
Associate Degree in Nursing (ADN) required, Bachelor Degree Nursing (BSN) preferred
Experience
Two years’ experience is preferred
Licensure/Certification
