Job description
Additional Information About the Role At Christian Hospital the Medical-Surgical and Telemetry service lines provide comprehensive care to young adult, adult, and geriatric patients (ages 18–90+) across multiple specialty-focused units. Across the 4th–10th floors, we offer medical oncology, adult surgical, neurology/orthopedics, renal, cardiac telemetry, and cardiothoracic surgery services. Telemetry monitoring is widely available, including advanced bedside monitoring in cardiothoracic surgery rooms.
Patient populations include post-operative surgical patients; neurological and neurosurgical cases; orthopedic and spine patients; renal conditions such as acute kidney injury, chronic kidney disease, and dialysis-dependent patients; and higher-acuity cardiac and pulmonary cases including MI, STEMI, TAVR, post-cardiac catheterization, CHF, arrhythmias, COPD, pneumonia, diabetes, and unstable stroke requiring telemetry monitoring.

HealthCare , six miles west of Christian, offers 24- hour emergency care, the Sleep Disorders Center and a variety of outpatient diagnostic and imaging services.
Preferred Qualifications Role Purpose Provides direct patient care activities including assessment, diagnosis, planning implementation, and evaluation within the guidelines of the standards of nursing care.
Responsibilities Promotes patient and family centered care in a healing environment.
Educates patients and their families on how to manage their illness or injury, including post treatment home care needs and medication administration. Participates in activities that promote patient safety, quality and regulatory compliance.
Participates in professional development. Develops, implements, and documents individual plans of care with defined goals in collaboration with other members of the interprofessional team and patient, family or caregiver in accordance with the established guidelines and standards of nursing care.
Proactively plans and ensures communication of the plan of care across the continuum of care. Uses critical nursing skills to assess and evaluate physical, psychosocial, and emotional needs according to standards of care. Assess patient preferences and barriers to involvement in care, including their values, emotional, spiritual, cultural, and population-specific needs.

Job Summary
Works collaboratively with an assigned panel of physicians to manage the patients specialized needs. The managing team does differ according to the chronic disease. Duties include assessment to identify member needs and development of specific care management plan to address needs. In conjunction with the Physician, implements care/treatment plan by coordinating access to health services across multiple providers/disciplines, monitors care, makes determination to arrange transportation and transfer patient if indicated, identifies cost-effective measures, makes recommendations for alternative levels of care and utilization of resources, promotes self-care management and ensures paper work is completed. Is an indirect caregiver. Complies with other duties as described. Must be able to work collaboratively with the Multidisciplinary team.
Essential Responsibilities
• Evaluates and identifies members needs.
• Interfaces with Primary Care Physicians, Specialists and various disciplines on the development of case management plans/programs.
• Monitors and evaluates the effectiveness of the case management plans and modifies as necessary.
• Coordinates the interdisciplinary approach to providing continuity of care, including utilization management, transfer coordination, discharge planning and obtaining all authorizations/approvals/transfers as needed for outside services for patients/families.
• Acts as a clinical liaison, per their specialty, with outside agencies such as County CCS, non-plan facilities, outside providers, employers and/or workers compensation carriers and third party administrators.
• Prepares reports, communicates program changes to appropriate staff and develops protocols in accordance with state regulations.
• Acts as a patient advocate and educator to assure that the patient has the knowledge to care for his/her condition and patient is educated and empowered to be responsible for participating in the plan of care.
• Develops individualized patient/family education plan focused on self-management, delivers patient/family education specific to a disease state.
