Youth Health Nursing Team Community Clinical Services
Permanent full-time role (1.0FTE), working 40 hours per week between Monday-Friday

About the Role

The Nurse Practitioner role is an expert nurse within Youth Health incorporating advanced knowledge and skills. In this role, you will practise both independently and in collaboration with other healthcare professionals to promote health, prevent disease and diagnose, assess and manage peoples health needs.

Key responsibilities include:

Provide care to rangatahi with a focus on assessing, diagnosing, treating, empowering and supporting patients/tūroro/tangata whai ora and their whānau using evidenced-based knowledge and skills to achieve optimal health outcomes.
Focus on complex patient/tūroro/tangata whai ora management to reduce the progression of disease, enable equity of access, promote wellness, limit disease, and prevent avoidable hospital admissions.
While autonomous in their practice, will work in collaboration with other providers including Māori health providers, Pacific health providers, other district health services and primary health care
Provide expert clinical leadership that builds a highly skilled nursing workforce and fosters supportive and collegial workplace culture.

About the Team/Service/Location

This is a Northlandwide position supporting a service that operates across the region. As part of a multidisciplinary team, youll work in a supportive and welcoming environment. The role is not restricted to Whangārei the successful candidate can be based anywhere in Northland, and we will locate the position where the best person for the role lives.

Our current team is spread across Kaitāia, Kerikeri and Whangārei, so we welcome applicants from all parts of the region.

Because this is a regional service, travel around Northland will be required, with the amount of travel varying depending on your home base.

The service provides holistic healthcare to ranagtahi aged between 12-24 years across the region. As well as school-based clinics the team provide static and mobile community clinics.

The Community Clinical Services, School Based Youth Service is tasked with providing Headdss Assessments in Decile 1-5 schools within the district alongside providing ****** health, mild to moderation mental health interventions and opportunistic vaccinations for in and out of school youth.

About you

Registration with the Nursing Council of NZ and with a current Annual Practicing Certificate as a Nurse Practitioner
A minimum of 4 years of experience in the specific area of practice
Clinical Masters degree or equivalent
A personal commitment to ongoing learning and development
Excellent presentation and communication skills (verbal and written) including report writing.
