Nutritionist for field counsellor

• Job description

Key areas of responsibility:
• Provide counselling related to product, its usage, side effects and treatment adherence.
• Provide counselling and psycho-social support to heart failure patients and their families.
• Develop individualized care plans and schedules to help manage their condition.
• Assist patients and their families with understanding their condition and how the product can help them Educate patients and families on lifestyle and medical management of heart failure.
• Coordinate with all stakeholders to ensure all aspects of patient care are addressed.
• Track patient progress, document patient care, and maintain patient records.
• Provide emotional support to family members and other caregivers
• Work with healthcare systems to ensure continuity of care for patients

Qualification:
• Bsc Food & Nutrition / Msc Food & Nutrition
• Language English(Mandatory)+ Hindi & Punjabi
