Clinical Care Manager

Position Overview
The Clinical Care Manager oversees all day-to-day clinical operations, ensuring patients receive timely, quality care. This leadership role provides direction, coordination, and accountability for staff performance, compliance, and service delivery. The Clinical Care Manager serves as the escalation point for patients, caregivers, and clinical staff while maintaining strict adherence to state and agency requirements.
Key Responsibilities
Daily Operations
Review and monitor patient schedules and caregiver assignments in Care Bravo.
Address staffing needs, service disruptions, and clinical escalations.
Ensure accurate and timely charting compliance across all field staff.
Supervise Clinical Care Coordinators (CCC) and Nursing Supervisors.
Weekly & Monthly Oversight
Lead weekly care coordination meetings with CCC, Nursing Supervisor, and Compliance.
Conduct chart audits, review staffing coverage, and monitor contingency planning.
Ensure timely completion of supervisory visits and caregiver evaluations.
Lead internal quality improvement (QI) sessions and prepare leadership reports.
Core Leadership Functions
Oversee patient intake, documentation, and payer authorization submissions.
Ensure service plans are completed, signed, and updated as needed.
Provide onboarding, development, and performance oversight for nursing staff.
Monitor compliance with HFRD, Medicaid, and internal agency policies.
Lead survey readiness, reporting, and performance improvement initiatives.
Required Qualifications:
Minimum 3–5 years of progressive leadership experience in healthcare, hospice, or related clinical care management.
Strong knowledge of Medicaid, GAPP, HFRD, and Georgia DCH regulations.
Proven experience supervising clinical teams, managing patient care authorizations, and maintaining compliance.
Proficiency with caregiver scheduling platforms preferred.
Exceptional leadership, organizational, and problem-solving skills.
Strong written and verbal communication, with the ability to manage escalations effectively.

Schedule: Full-time, Monday–Friday with availability for urgent escalations
