Your responsibilities will include conducting psychosocial assessments and providing interventions, referrals, social work, counseling, and follow-up for patients enrolled in assigned programs. You will also collaborate with the healthcare team to create proactive solutions and comprehensive treatment plans.

Essential Responsibilities:

Conduct thorough psychosocial assessments to identify client needs.
Develop and implement targeted interventions to support client well-being.
Partner with community organizations to facilitate referrals and connect clients with essential resources.
Provide individual, family, and group counseling and therapy sessions as needed.
Stay informed about best practices in psychosocial assessment and counseling theories.
