Clinical Care Coordinator

Bryan Health is committed to providing exceptional patient care, and we are seeking a skilled and compassionate Registered Nurse to join our team as a GIRMC Staff RN-Weekend Program. This critical role requires a strong commitment to delivering high-quality care, while collaborating with interdisciplinary teams to achieve optimal patient outcomes.

Key responsibilities of this position include:

Developing and Implementing Care Plans: Create personalized care plans that address the unique needs and goals of each patient, taking into account their medical history, social circumstances, and preferences.
Providing Education and Support: Educate patients and their families on healthy behaviors, disease management, and self-care strategies, empowering them to take an active role in their health and wellness.
Collaborating with Interdisciplinary Teams: Work closely with physicians, therapists, and other healthcare professionals to ensure seamless coordination of patient care, promoting efficient use of resources and optimizing outcomes.
Maintaining Accurate Records: Document all patient interactions, including assessment data, treatment plans, and progress notes, ensuring accurate and timely communication with healthcare teams.
