We are seeking a dedicated and compassionate Direct Support Worker (DSW) to join our team in providing exceptional care to individuals with developmental disabilities and other needs. The ideal candidate will have experience in various care settings, including assisted living, group homes, and nursing facilities. This role requires a strong commitment to patient observation, support with activities of daily living (ADLs), and the ability to work effectively with individuals facing challenges such as Alzheimer's and dementia.

Duties

Provide direct support to residents, assisting with daily living activities such as bathing, dressing, grooming, and meal preparation.
Monitor patients’ vital signs and overall health status, ensuring any changes are reported promptly.
Assist with medication administration according to care plans while adhering to HIPAA regulations.
Engage residents in meaningful activities that promote social interaction and cognitive engagement.
Maintain a clean and safe environment for residents by performing light housekeeping tasks.
Utilize specialized equipment such as Hoyer lifts when necessary for patient mobility.
Document care provided and any observations regarding residents’ conditions or behaviors accurately.
Collaborate with healthcare professionals to develop and implement individualized care plans tailored to each resident's needs.
Experience
Candidates should possess the following skills and experience:

First aid and CPR certification is preferred.
Experience working in home health, hospice care, or long-term care settings is highly desirable.
Familiarity with patient monitoring techniques and vital sign assessment.
Previous experience in caregiving roles involving individuals with autism, developmental disabilities, or memory care is a plus.
Strong communication skills and the ability to work effectively within a team-oriented environment.
